New Patient Registration
Registering with the Practice
We can only accept new patients who live within the practice boundary.
- You can register by completing a registration form obtained from reception or by downloading the forms via the links below
- Please bring with you proof of your address i.e. any letter confirming your address. You may also be asked for proof of identification
All patients, including children, are allocated a named, accountable doctor, referred to as your 'Usual GP'. We encourage you to see your Usual GP where possible, particularly for on-going problems.
New-patient registrations can take up to 7 days to be accepted on the Practice system.

New Patient Health Check
When you register, we will ask you to make an appointment to see either the practice nurse or health care assistant for a new patient health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.
