COVID -19 - Subject Access request

We may take longer than the statutory one month to respond to your Subject Access Request.  The Information commissioner recognises that there may be delays and responses may take longer than the statutory deadline.

With Flu season approaching we wanted to let you know that we will be starting the flu clinics in early September . Watch this space for more details closer to the time.
Please note our doors will be closed with immediate effect , this is to ensure the safety of all our staff and patients . If you have an appointment booked please call on arrival and and a member of staff will let you in.

 

New Patient Registration

Registering with the Practice

We can only accept new patients who live within the practice boundary.

  • You can register by completing a registration form obtained from reception or by downloading the forms via the links below
  • Due to the current situation we are asking that you do not attend the practice to hand these forms in , but to post them back to us.

All patients, including children, are allocated a named, accountable doctor, referred to as your 'Usual GP'. We encourage you to see your Usual GP where possible, particularly for on-going problems.

New-patient registrations can take up to 7 days to be accepted on the Practice system.
