New Patient Registration
Registering with the Practice
We can only accept new patients who live within the practice boundary.
- You can register by completing a registration form obtained from reception or by downloading the forms via the links below.
- Please bring with you proof of your address i.e. any letter confirming your address. You may also be asked for proof of identification.
All patients, including children, are allocated a named, accountable doctor, referred to as your 'Usual GP'. We encourage you to see your Usual GP where possible, particularly for on-going problems.

New Patient Health Check
When you register, we will ask you to make an appointment to see either the practice nurse or health care assistant for a new patient health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.
